Teen Health – New patient |
This visit is for women under the age of 18 who want to establish care. It will include a comprehensive review of her medical history, and an exam where appropriate based on care needs. During this visit we can review the menstrual cycle (physical and emotional), sex ed or symptom treatments based on the needs at the time. Referral may be needed, if in doubt, ask your PCP for a referral. |
Teen Health – return patient |
This visit is for established patients who are less than 18 years of age who need follow up care or problem related care. This may include medication follow up, STD testing, contraception changes, vaginal or hormonal symptom support. Referral may be needed, if in doubt, ask your PCP for a referral. |
Teen Health – counseling |
For patients who want more long term support on topics related to their health. Examples would be support on moods in the menstrual cycle, eating and supplements for health, one on one sex education etc |
General Women’s Health Care |

Women’s Health Exam – New Patient |
This visit includes a comprehensive health history review, full physical with breast and pelvic exam as indicated, review and collection of labs including pap smear if needed as well as health counseling based on current health needs. If you are new to the practice and have a health concern to discuss we can complete that with this appointment too. Topics may include: contraception and family planning, health screenings health goals, medical care for gyn health care needs. |
Women’s Heath exam- return patient, annual exam |
This visit includes a comprehensive health history review, full physical with breast and pelvic exam as indicated, review and collection of labs including pap smear if needed as well as health counseling based on current health needs. If you need support with new symptoms, and are established in care, but have not been seen for a year, you will chose this appointment for full review of care needs. Topics may include: contraception and family planning, health screenings health goals, medical care for gyn health care needs. |
Women’s Health – return patient, problem visit or follow up appointment |
These appointments are reserved for women who have been seen in the last 12 months, but have additional care need or follow up needs. Topics may include: urinary tract infection, medication adjustments or follow up, STD testing, vaginal odor or itching, breast changes, mastitis, other gyn problem topics that require follow up. Referral may be needed, if in doubt, ask your PCP for a referral. |
Contraception Counseling – New and Return pt |
During this visit we will review the full spectrum of contraception options from fertility charting/natural family planning, to hormonal options such as the pill, patch or ring or hormonal and non hormonal IUDs and implants. We can order IUD devices during this appoint to insert at a follow up appointment based on insurance coverage. This appointment is if you are not due for an annual exam, but want to discuss contraception. |
IUD (Intrauterine Device) Care |
These appointments are reserved for IUD insertion, follow up care and removal. |
Preconception Counseling |
Thinking about pregnancy? This visit is an opportunity to review health history and medical concerns related to planning your pregnancy. Topics may include fertility awareness, readiness for pregnancy, and genetic risks, supplements and lifestyle adjustments for a healthy you. We can also review birth care options in the region such as hospital, birth center vs home birth providers. |
Pregnancy Support |
While H&H no longer provides birth related care, you may choose to use consultation like services for your pregnancy. During these appointments you will have a basic assessment with fetal heat tone checks, maternal vitals, review birth location options, birth plans, pregnancy and birth advocacy support, review of newborn care services and as needed labs (such as confirmation of pregnancy support or genetic testing with Quest). |
SneakPeek |
Gender reveal blood draw - This test can be collected as early as 8-9 weeks, ideally once the pregnancy has been confirmed by ultrasound or doppler. The sample is mailed the day of the blood draw (for appt prior to 3pm), and results will be posted the following day. Using a small sample of mom’s blood, SneakPeek tests for the presence of male DNA (Y-chromosomes). If no Y-chromosomes are found, mom is carrying a girl. There is no requirement to be a client of mine for this test. |
Women’s Health - counseling |
This visit is reserved for times where you want further support with a non billable service. These appointments need pre-approval with Sarah prior to booking. These visits may include follow up care for symptoms of concern, work on learning about moods and symptoms in the menstrual cycle, work on boundaries, self care, nutrition and supplement support or other non medical counseling services. |
Perimenopause and menopause services |

Peri or postmenopausal annual exam - New patient |
This visit includes a comprehensive health history review, full physical with breast and pelvic exam as indicated, review and collection of labs including pap smear if needed as well as health counseling based on current health needs. If you are new to the practice and have a health concern to discuss we can complete that with this appointment too. Topics may include: discomforts of perimenopause such as hot flashes, insomnia, mood changes, vaginal dryness, pain with sex, hormone replacement therapies, health screenings health goals. |
Peri or postmenopausal Health Annual Exam - Established patient |
This visit includes a comprehensive health history review, full physical with breast and pelvic exam as indicated, review and collection of labs including pap smear if needed as well as health counseling based on current health needs. If you need support with new symptoms, and are established in care, but have not been seen for a year, you will chose this appointment for full review of care needs. Most insurances accepted including NH Medicaid and Medicare
Payment methods accepted: cash, check, Venmo, PayPal, Credit card and HAS card.
Referrals and pre-auths may be required with some insurances for problem visit like services. Check with your insurance, or ask you PCP for a referral regardless just to make sure your insurance can be used.
For those using insurance, billing will be processed first, and co-pays will be mailed to your home for payment.
Timely payment at time of the appointment without insurance is given a discount of 25%
